PATIENT DISCLOSURE, CONSENT & ASSUMPTION OF RISK

This plain-language disclosure is for patients using the WeGotDocs Platform. It is presented at account creation and before your first booking request, and your acceptance is recorded. It is part of, and incorporated into, the Terms and Conditions.

1. WHAT WEGOTDOCS IS — AND ISN'T

• WeGotDocs is a scheduling and search tool. We help you find healthcare providers and send them your appointment request.
• We are NOT a doctor's office, hospital, clinic, or medical group. We do not employ, supervise, or control any provider listed on the Platform.
• We do NOT give medical advice. Nothing on the Platform — including provider profiles, search results, rankings, or reviews — is medical advice or a recommendation that you see any particular provider.
• Your medical relationship is with your provider only. All decisions about your diagnosis, treatment, and care are between you and your provider.

2. EMERGENCIES

DO NOT USE WEGOTDOCS FOR MEDICAL EMERGENCIES. If you are experiencing a medical emergency — including chest pain, difficulty breathing, severe bleeding, signs of stroke, or thoughts of harming yourself or others — CALL 911 or go to the nearest emergency room immediately. The Platform is for routine appointment scheduling only and is not monitored for urgent medical needs. If you are in emotional distress or crisis, you can call or text 988 (Suicide & Crisis Lifeline) in the U.S.

3. WHAT YOU SHOULD KNOW ABOUT PROVIDER INFORMATION

• Provider profiles are written by the providers themselves. We perform a limited screening (checking that a license number appears active in the state database, exclusion-list screening, and NPI validation), but we do NOT verify education, training, board certification, malpractice history, hospital privileges, or quality of care.
• A profile on WeGotDocs is not an endorsement, certification, or guarantee of any provider.
• You should independently verify any provider through your state medical board (most have free public lookup tools) and your insurance company.
• Insurance information on profiles may be outdated. Always confirm coverage directly with the provider's office and your insurer before your visit. You are responsible for all charges from your provider.

4. WHAT YOU SHOULD KNOW ABOUT REVIEWS AND RATINGS

• Reviews are written by other patients and reflect their personal opinions and experiences. We do not verify the accuracy of any review.
• Star ratings are simple averages of patient scores. Search result order is based on neutral factors like location, availability, and specialty match — providers cannot pay for a higher rating.
• Do not rely on reviews as a substitute for your own judgment and independent verification.

5. BOOKING REQUESTS

• Submitting a booking request is a REQUEST only. Your appointment is not confirmed until the provider's office confirms it with you directly.
• We send your request to the provider by email. If you do not hear from the provider within a reasonable time, contact their office directly using the phone number on their profile.
• WeGotDocs is not responsible if a provider does not respond, declines your request, cancels, or is unavailable.

6. YOUR INFORMATION

The information you submit to book an appointment (your name, contact information, chosen provider, requested time, and any reason for visit you volunteer) is shared with the provider you selected so they can respond to you. We protect this information as described in our Privacy Policy and, where applicable, under HIPAA. Anything you choose to post publicly in a review is visible to everyone — do not include health details you want kept private, and never post someone else's health information.

7. ASSUMPTION OF RISK AND RELEASE

By using the Platform, you acknowledge and agree, to the maximum extent permitted by law, that:

• Your choice of provider is your own decision, made at your own risk, based on your own judgment and independent verification.
• WeGotDocs is not responsible for the acts, omissions, quality of care, conduct, availability, or billing practices of any provider, or for any injury, loss, or damages arising from care you receive or fail to receive from a provider you found through the Platform.
• You release WeGotDocs LLC and its officers, directors, employees, and agents from all claims arising out of the acts or omissions of any healthcare provider, EXCEPT to the extent caused by WeGotDocs's own gross negligence or willful misconduct or as otherwise non-waivable under applicable law.
• This is not a waiver of any claim you may have against a provider; your rights against providers are unaffected.

By clicking "I Agree" (or by submitting a booking request), you confirm that you have read and understood this disclosure, that you are at least 18 or a parent/guardian acting for a minor, and that you accept the Terms and Conditions and Privacy Policy.
